LA County Health Department Requirements


If you are going to be a Food Vendor at our event, we will need a signed copy of the LA Public Health Temporary Food Facility Application sent to us. You are also required to read and abide by the Health Code Requirements for Temporary Food Facilities. We have included these documents below with some helpful notes about each.

Each PDF file is stored as a Google Drive file that you can Print, Save, or Add to your own Google Drive. Here, first, is the document we need you to sign that we must submit to the Health Department:

✓ Required Application for all Food Vendors

  1. Open the file (click here)  in  Adobe Reader (free) or alternative PDF reader.
  2. Complete the document (fields can be typed-in using Adobe Reader)
  3. Print the Document and Sign the Signature Line.
  4. Scan the Document and Save it as a PNG, JPEG or PDF.
  5. Email the document to: info@reggaeveganfest.com as an attachment or send a link to the file. We’ll provide it to the Health Department.

  2. Please read the information provided for any requirements pertaining to you as food vendor such as booth structure, temperature, storage/display, employee practices, approved food sources, food handling, etc. Note: We will provide the required hand & dish washing facilities, trash/waste and employee toilets.
